Phase 1: Engagement & Trust-Building

Introducing the individual to available resources and support networks
Phase 2: Stabilization & Safety
Addressing immediate needs (e.g., housing, food, safety)
Connecting to detox or medical services if needed
Developing a personalized recovery and wellness plan
Providing mental health support and crisis intervention
Phase 3: Treatment & Recovery Support
Referrals to clinical treatment, outpatient, or residential services
Ongoing peer recovery coaching and mentorship
Participation in support groups (faith-based, trauma-informed, youth/family-focused)
Education on substance use, mental health, and emotional regulation
Phase 4: Skill-Building & Empowerment
